Evidence of hemodynamic derangement is often not always present at rest, necessitating dynamic
challenges to accentuate abnormalities.
Although cardiac catheterization remains the gold standard method for hemodynamic assessment, it is
limited by
practicality, access, risk, and its invasive nature; consequently, there is a need to better
understand noninvasive measures.
The etiology of exercise intolerance can be divided into 3 key physiologic components, as seen in Fig. 1. These components can coexist in the same patient and can vary within the same underlying condition due to the interaction with the duration of disease and comorbid conditions. Analyzing these mechanisms, in particular the interactions, may be challenging. Pseudo deficiency can inhibit interpretation, whereby 1 factor limits exercise capacity sufficiently to prevent another factor from reaching its physiologic potential.
